OSHA Training Purchase Order Program for MAVA Schools

To enroll in our purchase order program:

 

Create a purchase order that includes
the following information:


Billing Information:

• School name

• Mailing address

• Billing contact information, including name, phone and fax numbers,
   and e-mail address

• Billing name and address (if different)

• Indicate if the invoice can be sent by e-mail

 

Course Information:

• Note the number of initial authorizations by course:

      •$225 – 30-Hr Construction

      •$225 – 30-Hr General Industry

      •$525 – OSHA 510 Course

      •$525 – OSHA 511 Course


• Include if you would like the PO to remain “Open”

Upon receipt, we will issue you redemption codes by e-mail for the number of initial authorizations you have indicated.
Disperse these codes to students for course registration. Each code can only be used one time.

To redeem codes, students should visit www.MAVAOSHATraining.com , and then select the course they want to register for.
On the course page, students enter their redemption codes and click Register Now. This will enable students to register without being charged.
